Health and Safety at Work Act etc 1974; Management of Health and Safety at Work Regulations 1999; Control of Substances Hazardous to Health Regulations 2002 – Coronavirus COVID-19 

I am writing under the above regulations to inform you of my underlying health condition, which is XXXXXXX XXXXXXXX. According to advice from Public Health England, this may make me more vulnerable to the effects of COVID-19.  

As such I am formally requesting that you perform an individual risk assessment on me, to determine how I may be exposed to COVID-19 at work; how my health condition is impacted by  that exposure; and the protective measures that are needed to ensure my health so far as is reasonably practicable.  

Given my health circumstances, I request that you consult with an occupational health professional in performing the risk assessment, and that the advice you obtain from the medical professional is adhered to. I would note that this discussion should be about my specific health circumstances, rather than a generic discussion on workers with underlying conditions of the type that I experience.  

I am happy to speak to an occupational health provider under conditions of medical confidentiality to assist in the assessment process.  

I understand that a range of measures can be implemented to ensure my health, including the provision of physical barriers from work processes or colleagues; the enforcement of 2m social distancing site rules; the provision of Personal Protective Equipment; and my redeployment away from areas of exposure risk. I am sure all of these measures will be considered in your assessment to provide a safe system of work going forward.  

I have made my trade union the GMB aware of this request, and I will be remaining in close contact with them whilst the COVID-19 outbreak is ongoing. 

I hope to receive a response from you urgently, given the severe health and legal implications of COVID-19 in the workplace.
