Dear [name of your employer / HR manager/ line manager]

We are writing to raise a formal collective grievance.

Our grievance is in regards to the lack of PPE available to us to discharge our duties at work safely. We understand our employer is required to undertake a risk assessment under the Health and Safety at Work Act etc. 1974; Control of Substances Hazardous to Health Regulation 2002; Personal Protective Equipment Regulations 1992.

We are not confident that our exposure to risk in contracting Covid-19 is being minimised in our workplace and therefore we feel we have no alternative but to raise a grievance to ask this matter is resolved swiftly so that we can continue to discharge our duties with confidence that our health & safety is being protected at work.

We would be grateful if you could let us know when we can meet to talk about our grievance.

Due to the social distancing measures in place our union rep will be representing us collectively virtually and can arrange the necessary logistics for our hearing. My union reps details are: [name].

We await your response.

Yours sincerely,

[ names of those part of the grievance ]
